IRB Materials for Application

The following NMDP IRB applications can be downloaded by clicking on the links below. If you are unable to access the applications, please e-mail cjobe@nmdp.org.

• Application for Initial Review - Bio-Medical Studies
  This IRB application is to be completed for all bio-medical study protocols requiring initial review by the NMDP IRB. This application does not need to be completed if the protocol includes use of data from the NMDP research database and/or samples from the NMDP Research Sample Repository and there is no other involvement of human subjects. If this is the case, the below-listed application for research data and research samples must be completed instead of this application.

• Application for NMDP Research Sample and/or NMDP Research Data
  This application must be completed for all protocols to be reviewed by the NMDP IRB that only include data from the NMDP Research Database and/or samples from the NMDP Research Sample Repository. If the protocol involves human subjects in any way in addition to data from the Research Database or samples from the Research Repository, then the above-listed application must be completed instead of this one.

• Application for Initial Review - Social & Behavioral Studies
  This IRB application is to be completed for all social and behavioral study protocols requiring initial review by the NMDP IRB.

• NMDP IRB Request for Study Ammendment
  The "NMDP IRB Request for Study Amendment" form must be completed and submitted to the NMDP IRB if the study protocol and/or consent form is amended prior to the next NMDP IRB continuing review.
