How to Request Samples

1. Complete the Sample Request Form (PDF). There are three parts:
   • Requestor Information:
     Include complete contact information. This is important to the NMDP’s being able to process the request.
     
   • Type of Request:
     Select the type of request and (if applicable) the type of cells requested.
   • Description of Samples:
     Attach a typed or printed description of the sample being requested and the intended use.
2. Sign and date the request, stating that you have read and understood the way the samples may be used and agree to abide by the Research Policy.
   
   
3. Send the request to:
