How to Request Data

1. Complete the Data Request Form (PDF)

   There are three parts:

   • Requestor Information:
     Include complete contact information. This is important to the NMDP's being able to process the request.
     
     
   • Type of Request:
     Select the type of request and (if applicable) indicate how the data will be used.
     
     
   • Description of Data:
     Attach a typed or printed description of the data being requested.
   
   
   

2. Sign and date the request, stating that you have read and understood the way the data may be used and agree to abide by the Research Policy.

3. Send the request to:
